If you would like to find out what Nutritional Therapy, Allergy testing or TFT can do for you, you can make an appointment with me, please call 01636 816121. Free telephone and e.mail support is offered to all clients between appointments.
If you live at a distance or abroad I can also offer appointments using Skype (unfortunately this is type of consultation is not suitable for Allergy testing). Please contact me by telephone or email for details.
You will then need to fill in a detailed questionnaire and a week's diet diary. This diary is important as it provides me with vital information. You can download the questionnaire by clicking on the appropriate link below.

Please also print out the Client Care Agreement and bring it with you to your appointment for us both to sign.

Otherwise, post the forms to Gillian Key, 'Merrycoin', Ropewalk, Southwell, Nottinghamshire, NG25 0AL. (It might be worth keeping a copy.)
Cancellation policy - appointments cancelled with less than 24 hours notice will be charged, unless there are exceptional unforeseeable circumstances.If another booking is made, and kept within the next 2 weeks this fee will reduced by half. Please try to arrive on time. Should you be late, only the remaining time will be available to you.
